Thank you for your interest in Nativity Prep New Bedford for your student!  

The application process consists of completing two separate application forms, both through FACTS, which is the student information system used by Nativity Prep.  The first form covers student and family information, a records release form, and the input of two teachers’ names and email addresses for letters of recommendation.  The second form is the financial qualification form, which consists of a questionnaire and a secure upload of tax documents for custodial parents/guardians.

These forms are best completed on a computer.  You may start them on your own and reach out with any questions along the way.  You are also welcome to complete the application here at the school should you require further assistance.  The forms can be translated into Spanish if required as well.


Application

In order to begin the application, you will need to create a login.  You will find that information when you click the link.  Then, in order to complete the application, be sure to have the following information available:

  • Contact information for all custodial parents/guardians, including a frequently-checked email address.
  • Names and emails for two of your student’s teachers – one must be his current teacher, and one can be a recent former teacher.

If you have that information readily available, this part of the process should take 15-30 minutes.  Once you complete this application, we will be able to acquire your student’s school records and the two letters of recommendation on our own.  You will be able to return to this portal to check the status of your application.


Financial Qualification Form

After completing the application above, you will proceed to the financial qualification form, the link for which can be found here:  Financial Qualification Form

The link will take you to the FACTS Grant & Aid portal.  This form requires a separate login to be created and may take you a bit longer than the main application.  

Given the Nativity mission and as instructed by the Board of Trustees, the financial qualification for admission is that the student’s household – including all custodial parents/guardians who financially support the child – must be less than or equal to 60% of the state median income by household size.  A chart can be found by clicking here and looking at the far-right column.  The reason we are using the FACTS Grant & Aid form is so that household financial information can be processed by an independent third party in order to conduct a fair analysis to determine your family’s financial need.  

In order to complete this form, you will need the following information and documents:

  • Federal tax return documents Form 1040 for all custodial parents/guardians (required to upload)
  • W-2 Wage and Tax Statements for all custodial parents/guardians (required to upload)
  • An understanding of your monthly and annual expenses for the questionnaire
  • An understanding of the value of various accounts you may possess for the questionnaire

Timeline

We will begin evaluating applications starting March 15 once they are complete and all forms have been acquired, including school records and teacher recommendations. Then, we will continue reviewing and making decisions on a rolling basis.  We will receive information regarding your financial qualification approximately two weeks after you submit that form.  Application decisions are based on a complete review of the following:

  • Household Financial Qualification
  • Academic Reports (Report Cards & MCAS Scores)
  • Attendance Records
  • Conduct Records

As such, the timeline for us to provide a decision to you will be 2-4 weeks after all parts of the application process have been completed.  You will be notified of the decision by email.
